

Below is information you will need to upload to complete an online preschool application for your child. Eligibility for enrollment is determined through a ranking process with multiple factors including age, family income and family size.
REQUIRED DOCUMENTS
All documents listed below are required to be uploaded to the Online Application in order to complete the enrollment process.
- Birth Record for Enrolling Child
- Birth Record for All Siblings Age 18 and below Living in the Home
- Immunization Record for Enrolling Child
- Proof of Residency Dated Within the Last 30 Days
- Proof of Income for all Working Parents in the Home for the Most Recent 30 Days

DETERMINING FAMILY SIZE
Provide the names of all parents in the home as part of the application as well as the names and birth records for all siblings in the home who are under 18 years of age. Please refer to list of appropriate documents for Birth Record.

Birth Record for Enrolling Child, and
Birth Record for All Siblings Under 18 Years of Age Living in the Home
The birth record must indicate the child’s name, date of birth, and the relationship of the child to the parent/guardian. Siblings' information and birth records are required to establish family size.
Acceptable birth records include:
- Birth Certificate
- Birth Record from Hospital
- Baptismal Record
- School Records
- Medical Records
- Military Records
- Court Orders Regarding Custody
- Adoption Documents
- Records of Foster Care Placement Agreement
- County Welfare Department Records
- Certified Document from the U.S. Embassy that includes all family members

Immunizations Record (for enrolling student only)
Children entering preschool must have the following immunizations to be able to start preschool:
- 3 Polio
- 4 DTaP
- 3 Hepatitis B
- 1 Varicella
- 1 MMR (On or after 1st Birthday)
- 1 HIB (On or after 1st Birthday)

Proof of Residency
Provide current proof of residency showing parent/guardian's name, address and mail date within the last 30 days. Acceptable documentation may include but not limited to one or a combination of the following:
- Utility Bill (SDG&E, Cable/Internet, Water/Trash)
- Rental or Lease Agreement (Signed within the last 30 days)
- Mortgage Statement or Property Tax Statement
- Military Housing Order

Proof of Income
Proof of income must be provided by each working parent/guardian in the home for the most recent 30-consecutive days. The income from all sources should show gross family income (before taxes and other deductions).

Payroll Pay Advice or Pay Stubs Issued by Employer
Documents must have consecutive dates and show gross income for the last 30 days.

Provide all that applies:
- Submit 4 paystubs if paid weekly
- Submit 2 paystubs if paid bi-weekly or semi-monthly
- Submit 1 paystub if paid monthly

Self-Employment / Business Owner
Provide a signed and dated statement that includes the type of self-employment/business, estimated monthly income, frequency of pay, and method of payment along with supporting documentation listed on the right column. In order to deduct business expenses from income earned, evidence of payments such as receipts must be provided.

- Bank Statements
- Transaction or Trip Logs
- Business Logs
- Ledgers
- Receipts
- Any other business records that can show evidence of income/expenses

Cash Income
Provide Letter from Employer or Personal Written Statement.

The letter or statement should include the following information:
- Type of work done
- How often you get paid (weekly, bi-weekly, semi-monthly, monthly)
- Income amount for the current month
- How paid (in cash, personal check or electronic deposit)
- Name, signature and date

Student Financial Aid Letter
Amount of monetary assistance received by parent/guardian (such as grants and scholarships) to help pay for college or career school.

Provide the award letter for student financial aid which shows the itemized cost of attendance as listed below:
- Educational materials such as books, computer and supplies
- Tuition and fees
- Room and board
- Transportation
- Personal expenses, travel, miscellaneous

CalWORKs
Cash Aid. Go to https://www.mybenefitscalwin.org/ to access your account online.

Provide Verification of Benefits or Notice of Action issued by County Office of Health and Human Services that includes the following:
- Name
- Cash award amount
- Date within the last 30 days

CalFresh
Food Stamps. Go to https://www.mybenefitscalwin.org/ to access your account online.

Child Support
Court-ordered or mutually-agreed arrangement to financially support a child.

Provide child support award letter, court document with child support details, or personal written statement with the following information:
- Amount of child support received for the current month
- Signature and date, or record of date filed

Unemployment Benefits
Payment received from Employment Development Department (EDD). Go to https://edd.ca.gov/ to access your account online.

Provide Unemployment Benefits (EDD) document with transaction details showing the following:
- Name
- Recent dates
- Amount received

SSI
Supplemental Security Income
|
Provide SSI document or Bank Statement showing the following:
- Amount received for current month

Self-Declaration for Other Sources of Income
Provide a written statement of any other income received.
