California School Employee TB Risk Assessment
The Tuberculosis (TB) Risk Assessment Questionnaire is required by Human Resources every 4 years, unless you have a documented clear chest x-ray on file.
General Information:
- The questionnaire must be completed by a licensed nurse or physician.
- New hires must complete either a TB test or the risk assessment questionnaire within 60 days prior to hire.
You may need a TB skin test instead of the questionnaire if you:
- Have had close contact with someone with TB.
- Are experiencing TB symptoms, such as:
- Persistent cough
- Coughing up blood
- Night sweats
- Fever
- Unexplained weight loss
- Have traveled outside the U.S. for more than 1 month.
For School Site Employees:
Please schedule a time with your school nurse to complete the TB Risk Assessment Questionnaire.
For Central Office Employees:
TB Questionnaires will be completed by a Registered Nurse on the following dates and times:
Spring 2025 Dates:
- May 21: 1:00 – 2:30 PM
- June 4: 1:00 – 2:30 PM
- June 18: 1:00 – 2:30 PM
Starting July 1, 2025:
- Every 2nd and 4th Wednesday of the month
- Note: In November and December, only on the 2nd Wednesday
- Time: 1:00 – 2:30 PM
