Prescription Medications
If your student requires any type of medication during the school day, either on a daily basis, as needed basis, or for a short period of time only, we can only administer these medications at school with completed physician's recommendation for medication. This includes physician signature and parent signature. Please click on the link(s) below for the appropriate form(s) before a visit to your child's physician.
