ATHLETIC PACKET FOR THE 2016-2017 SCHOOL YEAR
Once the electronic athletic packet has been completed, print and sign the Confirmation Message. Bring the signed Confirmation Message along with the original signed Physical Examination Form and the signed Physical Evaluation History Form to the Athletic Director, Coach Josten. The process will then be complete and you will receive an email confirming that the student athlete has been cleared to participate in their chosen sport.

OTHER IMPORTANT INFORMATION
Thank you for your interest in University City High School Athletics. The following information will help you understand the seasons of sports and what you need to do in order to try-out and participate in highschool athletics. Most of our sports have a Varsity and a Junior Varsity level. Football, Basketball, Baseball and Girls' Volleyball all have Freshmen teams in addition to Varsity and Junior Varsity teams. All of our sports are C.I.F. sanctioned except Sand Volleyball.
You must have an Annual Physical that is dated after June 6, 2016, in order to try-out for any team. Since this is the first year that athletic packets will be processed electronically, the process for doing so has yet to be finalized. The website for submitting an athletic packet will not be up and running until Jun 7th. More information on electronic submission of athletic packets will be provided when available.
Your Athletic Packet must approved by the Athletic Director, Coach Josten. In order to help make processing your paperwork easier, please be sure to check the box for each sport that you plan on trying out fo rin the Fall, Winter, or Spring
The process for submitting, processing, and approving athletic packets should be improved. As can be expected with any "new" procedure, there will likely be some challenges that we will work through together.
