Transition Resources

What does transition mean?

 

The transition process is designed to help students with disabilities move smoothly from school to adult life. Transitioning to adulthood is difficult regardless of a youth’s abilities, so transition planning is key to ensuring success in adulthood for young people with disabilities. Adult services are very different from the services provided to students. There is no right to a job, day program or residence once your child leaves school, so early planning is essential!

Since students age out of the public school system at the end of the school year in which they turn 21, understanding the transition process is very important. Without a complete transition plan, your child could leave school without realistic goals for their future or without the skills needed to pursue further education, a job,needed community services, or independent living.


 
The student's case manager is the go to person for all things related to transition.  However, there are courses and other personnel to support a student's transition to life after high school.  Major components of on campus transition include: Curriculum Based Instruction (CBI), Transition Skills course, and/or consultation with a School to Work Transition Assistant.  Speak with your student's case manager for more information.

Community Based Instruction (CBI)

 

 

Community environments are visited repeatedly in order to provide instruction for target skills. Instructional procedures are determined and materials are secured prior to entering community sites. Modifications (to sites, instructional materials, instructional cues, etc.) are made, as appropriate, and opportunities are also made available for reinforcement and maintenance of mastered skills.

 

 

Transition Partnership Program (7366/7369)

 

 

The Transition Partnership Program (TPP) engages the CA State Department of Vocational Rehabilitation, with San Diego Unified School District and local business. The focus is to help a select number of students with an IEP, in their senior year who want to go straight to work after High School. Students typically work part time while continuing the education. Students will get assistance with job readiness skills, and may receive support for work supplies, clothing, uniforms, transportation, and job training

 

 

Transition Skills (7364/7365)

 

 

The Transition Skills class is designed to prepare students with an IEP for life after high school. Students may have the opportunity to learn about self-advocacy, self-determination career exploration, job readiness, employer expectations, healthy lifestyles such as fitness and nutrition and communications skills. All sections will be based on a person centered planning model and may be project based. Projects will help students acquire the skills consistent with current market place employer needs.

 

 

Work Incentive (7367/7368)

 

 

Work Incentive is an on-campus work experience program for students with an IEP, 14 years of age and older. This is typically the first work experience for many students. Students are given the opportunity to learn basic job and social skills that will help them prepare for a community- based job. At the conclusion of the class, students will earn 1 or 2 elective credits plus will be eligible for a professional reference for their resume. This course is included as part of the student’s school schedule.

 

 

Workability (7561/7562)

 

 

This course teaches students with an IEP vocational skills to help them transition from school to meaningful careers. Students are placed in a community-based paid or unpaid work experience. The student’s job performance will be monitored and evaluated by district personnel.
