All students enrolled in an Academy Program at Hoover are required to submit this form and initiate the exit process with their Counselor.Neither Academy Directors nor Counselors will make decisions regarding the request until this process is complete.

ONLY SCHOOL COUNSELORS AND HEALTH CENTER STAFF MAY SUBMIT REFERRAL FORMS FOR MENTAL HEALTH SERVICES.
Class schedules were made on the basis of YOUR course selections and alternative choices. Adjustments were made on the basis of summer school attendance and/or your grades in prerequisite courses. If an error has been made, please indicate what the error was. We will make every effort to correct it as soon as possible. You must attend all classes listed on your original schedule until a new schedule is issued.
