All students must be enrolled by a parent and provide the documents outlined below. Students are not allowed to attend class until the school has received all the required documents and proof that your child has received all currently due immunizations. (Page updated 8/18/15)

All students (Pre K–12)

  • Proof of residency. Can include items such as a recent utility bill, rental agreement or rent payment receipts, property tax payment receipt, mortgage documents, or military housing orders.
  • Proof of immunizations against polio, measles, mumps and rubella (MMR), diphtheria, tetanus and whooping cough (DTaP), hepatitis B (Hep B) and chickenpox (Varicella or proof of having had chickenpox).

New immunization requirements for 2016

Effective January 1, 2016, new California law SB 277 states that exemptions based on personal beliefs will no longer be an option for the vaccines that are currently required for entry into child care or school in California.  Most families will not be affected by the new law because their children have received all required vaccinations. Properly completed personal beliefs exemptions on file for a child already attending child care or school in California will remain valid until the child reaches the next immunization checkpoint at kindergarten (including transitional kindergarten) or 7th grade.  Personal belief exemptions filed from another state or country are not valid if student is transferring to a California school after January 1, 2016.  Properly completed medical exemptions for immunizations will be allowed for  students when they have been issued by a licensed physician.

Preschool

Children entering Preschool or child care must have the following immunizations:

Transitional Kindergarten

  • Transitional Kindergarten Website

  • Required for all students who turn five (5) years between Sep. 2 and Dec. 2, 2016 for the 2016-17 school year.
  • Copy of a birth certificate, baptismal certificate or passport.

  • Children entering transitional kindergarten should have:
    • 5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday)
    • 4 Polio (3 doses meet requirement if at least one was given on or after the 4th birthday)
    • 3 Hep B
    • 2 MMR (Both doses given on or after 1st birthday. Only on dose of mumps and rubella vaccines are required if given separately.)
    • 1 Varicella

Kindergarten

  • Children must be five (5) years or older on or before Sep. 1, 2016,  in order to be eligible to attend kindergarten for the 2016-17 school year.
  • Copy of a birth certificate, baptismal certificate or passport.
  • Children entering kindergarten should have:
    • 5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday)
    • 4 Polio (3 doses meet requirement if at least one was given on or after the 4th birthday)
    • 3 Hep B
    • 2 MMR (Both doses given on or after 1st birthday. Only on dose of mumps and rubella vaccines are required if given separately.)
    • 1 Varicella

Physical Exam

California’s Child Health and Disability Prevention (CHDP) program requires every child to have a physical examination before entering school in kindergarten. Please submit a copy of your child's medical checkup to the school when you register for kindergarten or as soon as it is completed during kindergarten.

You are encouraged to obtain this exam by kindergarten entry to make sure your child is healthy and ready to learn. Transitional Kindergarten students who submit a physical exam will fulfill this requirement for kindergarten.

Students who have not met this requirement by the appropriate date may be excluded from school attendance for up to five days. Parents/guardians needing assistance meeting the requirements can call 1-800-675-2229. You are encouraged to complete this exam at kindergarten entry to make sure your child is healthy and ready to learn.

K-1 Dental Exam (OHA)

Effective January 1, 2007, an Oral Health Assessment (dental examination) by a licensed California dental health professional is required for children entering public school for the first time (at kindergarten or first grade). The Oral Health Assessment must be completed and returned to the school by May 31 of that first school year. Assessments done 12 months prior to school entry also meet this requirement. Obtain the form from the school or your dental health provider to complete.

Parents/guardians may sign a waiver of this requirement, which is found on the OHA form available at the school. There is no penalty or exclusion if this requirement is not met. The parent/guardian can call 800-675-2229 for assistance in finding a provider for the Oral Health Assessment.

All Grade 1 students

Must be at least 6 years old by Sep. 1 unless they have completed a year of public school kindergarten.

All Grade 7-12 students

  • Children entering grade 7 should have:
    • 4 DTaP (3 doses OK if last dose was given on or after 2nd birthday. At least 1 dose must be Tdap or DTaP/DTP given on or after 7th birthday for all 7th-12th graders)
    • 4 Polio (3 doses meet requirement if at least one was given on or after the 2nd birthday)
    • 1 MMR (Dose given on or after 1st birthday. Mumps vaccine is not required if given separately.)
    • Varicella (1 dose for ages 7-12; 2 doses for ages 13-17)
  • Children entering grades 7-12 should have:
    • All of the above
    • 1 Tdap booster shot given after the 7th birthday.
    • 2 MMR (Or any measles-containing vaccine. Both doses given on or after 1st birthday)
